I don't see obesity epidemic and body image epidemic as mutually exclusive. They can both exist and probably do. I do agree that obesity is over diagnosed and that even doctors get tunnel vision. At the other end, I've also heard stories about women being dismissed by doctors who see them as too thin and thus not looking for other causes of their ailments.
